Certificates of Enrollment
2011-2012 Certificates of enrollment instructions:
A limited number of COEs will be available at our August and September FOCHE members meeting. (First come, first served) . Please contact tcunningham@mde.k12.ms.us or 662-349-6552 to request your Certificate of Enrollment card.
- You'll need to complete one copy for each child of six years of age and older. (Kindergarten is not mandatory in MS.)
- Year is 2011-2012
- The student's SS# is neither requested nor required. Do NOT include.
- On the form, HSLDA suggests Description of Home Instructional Program be worded with ONLY the following: " Home education program" You do not need to disclose a specific course of study nor curriculum nor anything else.
- Complete the remainder of the form for each pupil. Parent or guardian must SIGN. MAKE A COPY FOR YOUR RECORDS!
