REFERRAL INFORMATION
|
1. The child has demonstrated delays in one or more of the following developmental ares: attachment, emotional, social, cognitive, self-concept, self-help, behavioral, receptive/expressive language, and physical development.
The child must meet at least one of the following criteria to qualify.
1. The child is unable to attend regular child care due to substantial developmental or behavioral problems. 2. The child has been exposed to substantiated cases of abuse/neglect with development or behavior problems.
3. The child is in imminent danger of being removed from the home.
